Standard Forms for Safe Streets and Roads for All Grants
Linked on this page are the Standard Form (SF) 424 family forms that are required application materials for the Safe Streets and Roads for All (SS4A) discretionary grant program to detail proposed funding, project, and lobbying information.

For all SF Forms, we recommend downloading each form and saving it to your computer before completing it. These fillable forms generally cannot be opened or filled out in a browser; instead, please use any PDF reader, like the free Adobe Acrobat reader. The files on this page and in Valid Eval have been formatted to allow for digital signatures such as Adobe digital signatures or Docusign. Simply typing your name in any signature line will not be accepted.
Note: You may notice the SF-424 General Application for Federal Assistance expired in November 2025. This is the latest version at the time of the NOFO release date. Applicants will be notified if a new version becomes available, and new links will be posted on the SS4A website and Valid Eval. The OMB number and expiration date of the most up-to-date versions of each form are included below.
