SMART Federal Financial Reporting: SF-425

SMART Grant recipients should use the SF-425 form for their federal financial reporting. Forms should be submitted quarterly, even in quarters where no reimbursement is being sought (enter '0' on the appropriate lines). Email completed forms to smartreports@dot.gov. Users should download the form and save it to their computer before completing it.
