DRC Accommodation Request Form

The DRC uses an Accommodation Request Form to help better understand employee requests for reasonable accommodations.

To complete the form:

1. Download it to your computer and take note of where it is stored.
2. Open the file using Adobe Acrobat Software.
3. Please complete all fields. 
4. Click the "submit" button to generate an email that will submit the form to the DRC. 

NOTE:  You must complete the form in Adobe Acrobat, and not while viewing it in your browser. If you try to fill out the form while in the browser, the "submit" button will not work.
